The following form is intended for use by referring doctors:
Doctors may fax referrals and indicate if they would like our receptionists to forward appointments direct to their patients or whether the patient will telephone for the appointment.
If the appointment is urgent, the doctor should telephone our receptionist or contact the cardiologist. For faxed referrals, the doctors could mark the referral as “urgent”.
